Step 1: We inform your patients of your CCM program

We identify patients who qualify, and follow up with an enrollment call.

Step 2: We talk to your patients each month

Enrolled patients receive monthly phone calls from their dedicated Care Manager who understands their current medical challenges. Calls are personalized and focus on patient's changing needs and help them overcome barriers to their care.

Step 3: We report back to you

We send you notes from each call. Your providers have more constructive conversations with their patients during office visits because they have more comprehensive information about the patient's health.
